B.   Minimum Qualification Standards
  Education : Bachelor’s Degree in Social Work
  Training : 4 hours of relevant training
  Experience : 1 year relevant experience
  Eligibility : RA1080 (Registered Social Worker)
C.   Preferred Qualifications
  Education : BS in Social Work preferably with MS in Social Work
  Training : 4 hours of training in adoption/counseling/case interviewing/ case writing/communication/ case management
  Experience : 1 year in handling case management
  Eligibility : RA1080 (Registered Social Worker)
D.   Job Summary
    Under general supervision and with some latitude for exercise of independent judgement, performs somewhat difficult, responsible professional work pertaining case management requiring training, some experience and broad knowledge on adoption and foster care and does other related task that may be assigned from time to time.
E.    Job Functions
  1. Implement policies and work plans established by the agency and render direct service in accordance with the functions and objectives of the agency within area of assignment;
  2. Prepare/write project proposal/case study reports of client’s served;
  3. Implement case management to regular clients;
  4. Provide casework services and other assistance needed by the clients/families;
  5. Conduct intake interview and accomplish forms that will determine client’s eligibility for services/programs;
  6. Conduct home visits and collateral contacts/interviews to identify factors affecting clients’ adjustments;
  7. Refer client/clients’ families to other agencies and make follow ups and evaluate benefits and assistance received;
  8. Coordinate with SWAD/ Hospitals/ Funeral/ NGOs/LGUs on the aftercare services for clients and access to other services;
  9. Prepare clients for their return to their respective families/communities;
  10. Establish and maintain good working relationship with GOs and NGOs and other stakeholders;
  11. Attend and coordinate with LGUs on disaster related incidents and access appropriate augmentation on disaster response services;
  12. Maintain and update recording on client’s status and progress;
  13. Prepare monthly and quarterly reports, feedback reports, reply to communications received;
  14. Attendance to regular monthly meeting of the unit;
  15. Perform other related task as may be assigned.
